1.初診：請先填初診表，再進行預約。
2.預約：

請由「官方LINE 」進行預約。
請在訊息中載明「家長真實姓名」、「毛孩姓名」、「手機」、「欲看診內容」、「欲看診時間」。

3.待收到訊息後，宅宅將儘速與您聯絡。

1. First Visit：Please fill in this form, then start to make reservation.
2. Make an appointment：

You can either make an appointment through our 'LINE '
Please write for us 'your name,' 'your pets names,' 'cellphone numbers,' 'what happened to your pets,' and 'when will be available for you' in the message.
After we received your message, The MobileVet Clinic will contact with you soon.
